RMA Warranty Request

Use this form only to request return authorization for IGT material that is still under warranty and is being returned because of a defect, failure, or other approved warranty condition. Provide the sales order number, part number, quantity, serial number, and reason for return so IGT can evaluate and process the request.

Do not use this form to order replacement parts or request a paid repair.
For replacement parts, use the Gaming Parts & Support to email csfaxlv@igt.com or call 866-777-8448.

For paid, out-of-warranty repair, refurbishment, or component-level service, use the Repair Depot Paid Repair Request section on IGT.com.
